How To Request Degree Verification:

Step 1 - Please obtain the following forms:

• Verification form (with graduate’s information completed)
• Signed release of information authorization

Step 2 - Send your completed forms using one of the following options. Note: Please include your full contact information in all correspondence.

Step 3 - We will process your request within five to seven business days and forward the completed forms to one of the following, as requested:

• Florida Board of Nursing
• Other state’s Board of Nursing
• Potential/current employer
